Metronomic TMZ

Hi, just posting this in order to know if someone else is on Metronomic TMZ too. My wife has started 2 weeks ago with this protocol (50mg/m2) also concurrent with Avastin 15 mg/Kg/21d. Since the first infusion she got improvements on her left side, she has to deal with some level of hemiparesia after surgery on Oct-19

Would like to know if someone else is experiencing fatigue, tiredness or lack energy  due to the accumulation of TMZ in this schedule, or maybe the window between Avastin's infusions is too big (21d) and she is not able to keep a stable level in blood and the effect goes off after 10/14 days.

  1. Forgot to mention she faced a fistula last week, drained CSF for some days, but now is under control with acetazolamide and pressure bandage. Maybe I can create a post asking about fistulas, will do.

  2. Francisco, did your wife start Avastin at the same time as starting TMZ? Alan (hubby) has f/n Avastin infusions his dosage is 8.5mg/Kg/f/n which causes some fatigue.

    2. Hi Linda, yes she started both at the same day. As this is the first cycle taking them we are learning about the side effects. Did he feel better in the days close to infusions?

  3. Hi Francisco, Alan feels more tired on the day of the infusion and usually for a day or two afterwards. I thought the usual dosage for GBM avastin was 10mg/k on a 2 or 3 week bases and sometimes as low as 5mg/k/fn bases. 15mg/k seems a little high from what I've heard. From what I understand the higher the Avastin dosage the more likelihood that a perforation may occur.

    Hi Stephen, that is correct every 2 weeks Alan has iv Avastin. Actually the oncologist suggested we change to 10mg/k on a 3 weekly schedule but we declined. It would be more convenient but as he is tolerating it very well and MRI's have been stable for 14 months we thought we'd leave as is. I'm wary of Avastin my sister was using it for ovarian cancer and had a bowel perforation.

  4. Hi Francisco.

    When does he take his chemo? I used to take it first thing in the morning and it lead to horrible fatique and nausea. Between lunch and dinner -same result.
    I finally started taking it before bed and it changed everything. I sleep through side effects. I have to skip dinner in order to have my stomach empty.

    1. Hi Amber,
      She is taking it before bed, is working well for her. We use to have dinner at 9/9.30pm, then around 11pm she takes Melatonin 20mg, Ondasentron 8mg, Verapamil 240mg and finally at midnight 80mg TMZ + Keppra 1g. Recall that 3 years ago when she was diagnosed with AAIII, she did the protocol 5/23, taking the TMZ just before bed and worked well too.
