Cocktail question, Fluoxetine or Celebrex?

Dear Stephen and all,

I have a question concerning my girlfriend's father. He is 57 years old. He was diagnosed with a Grade IV, MGMT unmethylated Glioblastoma in the left occipital lobe, September 2018. After the surgery, he received radiotherapy and chemotherapy using VAL-083 in China during 08/11/2018 - 27/06/2019. He had an MRI scan on 20/08/2019 and there is a 17mm*10mm GBM recurrence in the left temporal lobe.

Now his oncologist suggests receiving the standard 5/23 Temozolomide treatment. Since his GBM is MGMT unmethylated, we want to have the cocktail to enhance the performance of Temozolomide for him. He is taking Metformin, Verapamil, Chloroquine and Keppra. We want to choose 1 more medicine from Fluoxetine and Celebrex, which one is better?

Details of the condition of my father are listed below:

  • On 28/09/2018 he had surgery due to a tumor in the left occipital lobe. According to the biopsy, he was diagnosed with a Grade IV Glioblastoma Multiforme, MGMT unmethylated, ki67 70%, IDH1/2(-), 1p/19q non-codeleted, non-diffuse.
  • On 08/11/2018, he began to receive radiotherapy and chemotherapy using VAL-083 with a dose of 30mg/m2 IV. But soon he suffered severe myelosuppression and both radiotherapy and chemotherapy are stopped for 2 weeks. After that, the dose of VAL-083 was tuned to 20mg/m2.
  • On 24/06/2019, the assessment via MRI is CR I class, complete response (that shows the VAL-083 is effective for him)
  • On 27/06/2019, he finished the clinical trial, including 10 cycles of treatment using VAL-083.
  • On 20/08/2019, MRI shows that there is a 17mm*10mm enhanced mass in the left temporal lobe (the original tumor was in left occipital lobe).

Questions on Chloroquine for my mom

Hi folks/Stephen,

My mom is a GBM patient who is EGFRvIII amplified, and basis my research and Stephen's inputs, I realised that Chloroquine is a great adjuvant for my mom. I have been reading up on Chloroquine and had the following questions on the same:

1. I am not able to find Chloroquine in my country, but Chloroquine phosphate instead. Can they both be used interchangeably? Is the efficacy of both just as good?

Basis my research, the dosage of Chloroquine should be 150 mg. What should be the dosage of Chloroquine Phosphate, if it can be used interchangeably?

2. I also read that Chloroquine is a chemo sensitiser, but couldn't find any information on what the timing of this drug should be for maximum efficacy.

My mom takes lomustine+temozolomide in the morning. Can you please help me on when the Chloroquine/Chloroquine Phosphate should be timed for maximum efficacy?

3. How long should chloroquine be taken for? Should it just be done till the chemotherapy or should it be done post the chemotherapy too? I see different researches doing this protocol differently.

My mom has completed 3 cycles of temozolomide and 4 cycles of temozolomide+lomustine. We plan to do two more cycles of temozolomide+lomustine and stop the chemo post that.

4. My mom is also doing the supplements in the table below.

a. My major concern is Artemisia since it is an anti malarial drug too. Is it safe to do both? If yes, how I should I time them so that I get the efficacy of both for my mom?

b. Is there any drug reaction that I should be concerned of? I researched on drugs.com for the interaction list, but all that I could find were two reactions since most of the naturopathic supplements were not there in their database(Interaction list for my mom is here). The two reactions that they talk of is reaction between Chloroquine and Keppra, Chloroquine and Celebrex, how big a concern are they?

Celebrex post-Avastin?

Hi all,

My brother received his first Avastin treatments over the past few months and suffered a brain bleed. After he was stabilized, his NO said we needed to take a break from Avastin and re-evaluate after 1 month. James was also on Xarelto during this Avastin treatment due to a blood clot that formed in his leg. He is no longer taking Xarelto.

James desperately wants to get off of steroids and is beginning to dose down. He's previously done OK dosing down slowly, but I wanted to ask about introducing Celebrex.

I'm concerned that maybe it could be a bad idea due to the brain bleed? The bleed has not fully "absorbed" so it is still present, though it is under control.

Any previous experiences or references would be greatly appreciated.

Cocktail review after 1 year

Dear community,

I'd like to revise my husband's cocktail after 1 year of doing this protocol. He is at 14.5 months from diagnosis and has just started his 12th cycle of TMZ and just got his 3rd excellent MRI saying that the main part of the thalamic tumor is barely visible at the moment (which was the bigger concern of the two tumor sites) and the frontal one is still non-enhancing. 

Although he is methylated, I attribute the good results to the cocktail approach or at least to the synergy between the chemo and the cocktail because the first MRI after radiochemotherapy showed progression and it couldn't be pseudo progression, at least not on the frontal site because only the thalamic site was involved in the treatment. 

We're reaching the 1 year mark of supplementation with the following: 

Fluoxetin 40 mg
Chloroquine phosphate (Delagil) 250 mg (he already stopped taking it)
DCA  500 mg 2 times a day for a patient who is sadly weighs only 55-58 kg these days
Silymarin 630 mg
Celebrex 400 mg

The rest of the meds were added later on gradually. 

In your opinion, is it good to stick to the cocktail if it's working because we don't know what elements are working or after a year would it be wiser to make some changes in the regimen to disturb the tumor?

Does it make sense to take Celebrex if there's no edema and we are 1 year from radiation therapy?

I'm thinking about replacing DCA with the ALA + HCC protocol. 

Sorry if it was discussed before but is LDN an antagonist to methadon? Can the ALA + HCC protocol be effective without LDN? Combining this protocol with ketogenic diet is not an option for us because he has a sweet tooth and these days his main calorie intake is from fruits and baked goods which makes me frustrated big time but I can't help it, he lost so much weight. 

I'm not sure if we can use 800 mg of ALA 2 times a day instead of RLA 800 mg 2 times a day or should we double the dose of ALA considering that ALA is just 50% RLA.   

What's the longest period of time that a patient can take DCA? My husband is on a fairly low dose although he noticed considerable neuropathy on several occasions when we ran out of methadone for one day. Since it has painkiller effects I suppose that the neuropathy is continuous but methadone may conceal DCA's side effects. So I suppose that after a year we should address this problem and make at least a longer break of DCA.

Since his seizure he is on 300 mg of valproate acid 3 times a day, too. Considering that it became a necessity to take an AED I see reason to maybe replace fluoxetine with LITHIUM and so he would be taking almost the whole CLOVA cocktail. He's been on cimetidine 800 mg since last Sept./Oct. I'd like to exclude olanzapine because I read terrible things about its side effects but this way he is already taking 3 of the 4 medicine of the CLOVA cocktail anyway. 

By the way, at first, he was prescribed the same amount of sodium valproate in the A&E but the NO changed it to valproate acid later on. I know that those two are in the same family but can be any difference between their tumor-fighting properties?

I read here an older comment that stated that a patient's NO advised against taking Silymarin and valproate together because valproate is metabolized by the liver and Silymarin flushes it out. Is it a true statement in your opinion? I'm not sure if we can keep that in the cocktail. 

Cimetidine: I suppose it's advisable to take it together with TMZ. In our case it will be 24 cycles if he can tolerate it on the long term. So should he continue it for an additional year?

If stopping with DCA, Celebrex, chloroquine, he will only take a few prescription drugs (metformin, fluoxetine or lithium, alfacalcidol, D,L methadone and of course the anticonvulsant) and it seems a bit scary that natural supplements will be predominant. 

Do you recommend to add anything to this list at this stage of the disease?

Can Celecoxib and Ibuprofen be used interchangeably?

Hi folks/Stephen,

My mother is a grade 4 brain cancer patient. We have been using Celebrex(Celecoxib) as a part of her drug cocktail.

However, in situations of heavy head/headaches and/or body aches, she often asks for Ibuprofen which we've observed helps relieve her pain.

Both being COX-2 inhibitors, I wanted to ask if both Ibuprofen and Celebrex can be used interchangeably, or is there some reaction that I should know of? If there is a reaction between the two, how much difference in time should I keep between the two medicines?

Would love to hear from you guys.

Need suggestions to incorporate supplements in my mom's cocktail(GBM Patient)

Hi folks,

My mother(Age 47) was diagnosed with glioblastoma(IDH1/2 -ve, Methylated) in September 2017, and it has left me devastated ever since. She has completed her 6 weeks of radiation/chemo and her first cycle of 5/23 temozolomide. Her next cycle of temozolomide starts 5 days from now. I stumbled upon this blog just a week back, and have found this to be immensely helpful and resourceful.

I need help from you guys on what else should I incorporate in my mom's cocktail. Following are the supplements that are present in her cocktail per day already:

Metformin: 500 mg
Boswellia Serratta: 4000 mg
Resveratrol: 400 mg
Fish Oil: 3600 mg DHA+EPA + 400 mg of other omega 3 fatty acids
Curcumin with piperine: 5000 mg
Longvida: 400 mg
Quercetin: 5000 mg
Melatonin: 20 mg
Selinium: 200 mcg
Vitamin ADK supplement with 5000 IU Vitamin D3
Ashwagandha: 500 mg
Garlic: 6 cloves
Ginger: 6 cloves
Dendritic cell therapy: On the cards
Cannabis oil: Dropped because it was suppressing her WBCs

She is also on ketogenic diet and has done about 2 weeks of hyperbaric oxygen. We are looking at two more weeks of hyperbaric oxygen.

I'm looking for more supplements that are good adjuvants to temozolomide/standalone good adjuvants that I should definitely include in my mom's cocktail. It'd be great if you guys could help out in more supplements that I should include, I'm finding it very hard to self medicate my mom. Basis Ben Williams' book and reading several blogs, the following supplements have repeatedly been catching my attention:

1. DCA
2. Chloroquine
3. Care oncology clinic protocol(Metformin + Doxycline + Mobendezole + Atorvastatin)
4. Ruta 6 + Calceria Phos(Not sure if it is a good idea to use while on chemo)
5. Methadone
6. Veramapil
7. Low Dose Naltrexone
8. Disulfiram
9. Perilyl Alcohol
10. Methadone
11. Celebrex

She is already taking a total of 50 meds per day and I think she can take only 10-15 more. Would be great if you can help me from above list/any other supplement that should definitely be a part of her cocktail.

Treatment options post-RT/TMZ phase for IDH1 mutated, MGMT unmethylated GBM

Hi all,

I was diagnosed in late September with a GBM (frontal, left, with large cyst, IDH1 mutated, MGMT unmethylated), which was subsequently successfully operated (gross total resection) at the end of September. I guess as many/most here, I eventually stumbled across Ben William's book, the Glioblastoma Treatment options guide and ultimately this invaluable blog and community, which I have been studying and following closely since. I recently concluded the first phase of my treatment, following standard Stupp Protocol (6 weeks concomitant RT/TMZ) and am currently planning next steps (plus waiting for first post-RT MRI next week...).

While I did not get 'smart' in time to save my tumor material from being paraffined post-OP (unbelievably, this is still standard practice here in Germany in most hospitals), I did actively supplement my first phase of treatment with what I think is a reasonably aggressive 'cocktail' approach, including the following components:

Meds:
- Chloroquine, 1x 250mg
- Celebrex, 2x 200mg
- Disulfiram, 1x 250mg - 500mg (+4mg copper)
- Sativex (THC/CBD spray), ca. 3-4 sprays (approx. 15-20mg)

In general, I tolerated these medications without any major problems or side effects, with a few exceptions. Notably, towards the end of the treatment I developed some peripheral neuropathy in my left foot, which has now almost recovered, however (took around 3-4 weeks to recover). Nevertheless, it cause me to cease the Chloroquine and Disulfiram shortly before the end of my RT treatment phase. In addition, I found it a little hard to tolerate Sativex as I wasn't too keen on the psychoactive effect, which gave me some anxiety / mild panic attacks from time to time at night. As a result, I took it only for around 3 weeks or so.

Supplements:
- Berberine: 1000mg
- Boswellia Serrata: up to 4400mg (gradually increased dosage over course of RT to protect from Edema)
- CBD oil (8%), 5 drops (started after ceasing to take Sativex)
- PSP, 2100mg
- Curcumin (Longvida), 2000mg, increased to 3000mg towards end of treatment
- Green Tea Extract, 3625mg
- Lycopene, 20mg
- Matiake D-Fraction Pro, 65mg (3x 23 drops)
- Melatonin, 20mg
- Omega 3 DHA/EPA, 3528mg
- Probiotics, ca. 40bn units
- Pterstilbene, 250mg
- Resveratrol, 500mg
- Selenium, 200ug
- Silymarin, 1500mg
- Soy extract, 3750mg
- Vitamin D, 9000IU

In general, all of the above were well tolerated without side effects. I'd also like to mention I was able to avoid any kind of Edema / Cortisone use during my RT therapy, which I believe may have been at least in part facilitated by Boswellia in combination with Celebrex.


Other:
- Ketogenic diet, max 40 g Carbs per day; generally constant medium to high Ketone bodies when measuring. Started 1 week before RT, and continued to last day
- Caloric restriction, lost ca. 8 kilos in 6.5 weeks of RT, which I think equates approx. 600kcal or so in daily caloric restriction
- Daily morning smoothie, with variety of hopefully beneficial things like berries, broccoli sprouts, spirulina, tumeric powder, Matcha green tea, cocoa powder,  etc.
- Daily walks of ca. 1 hour to combat radiotherapy fatigue and keep fit

Ketogenic diet was somewhat difficult to maintain psychologically, but possible due to my partner's kind help in continuously seeking out new and often tasty dishes to keep things interesting. Caloric restriction much easier, since Temodal anyway caused me lack of appetite. I believe daily walks were very helpful to avoid RT fatigue, which affected me only in very minor way and much less than I expected.

NEXT STEPS & QUESTIONS

I am currently considering next steps, and having talked to various NOs and other Brain Tumor specialists, I am still not entirely convinced what the right way forward is. As expected, most doctors do not want to deviate from the Stupp Protocol (i.e. follow up the RT/TMZ phase with 6 months of 5/23 TMZ cycles). However, I am not convinced such a treatment would necessarily add much benefit in my case, since my tumor is MGMT unmethylated.

One of the leading specialists in Germany told me that the unmethylated MGMT status is irrelevant in the case of IDH1 mutated tumors like mine, since a study (NOA-4) showed that there was no significant difference in responsiveness  between MGMT methylated or unmethylated IDH1 tumors.

However, upon further research I stumbled across the following interesting study from China, which seems to suggest that IDH1 mutated tumors might in fact be particularly resistant to TMZ (3-10x more resistant in cell culture test). The study also notes that in China they observed relatively little additional benefit of TMZ cycles for the IDH1 mutated group of patients compared to RT alone, and the authors argue that survival benefits for IDH1 mutated tumors may simply be the result of a less invasive / more benign type of tumor relative to wildtype. It makes me wonder if the fact that MGMT doesn't seemingly play as big a role for IDH1 mutated tumors is simply the result of the fact that neither responds well to TMZ...:

I'm therefore a bit hesitant to simply go ahead with TMZ therapy hoping for the best, and would like to consider other options.
One approach I am considering is Immunotherapy at the IOZK clinic in Cologne, which is not far from my house. However, because I don't have frozen tumor material, they would have to make a personalized vaccine using a liquid biopsy approach. This, in turn, could make the treatment even more unproven than vaccines anyway are even when made from tumor lysate. An additional option which could possible materialize down the road (but not yet, as no trials are running here presently to my knowledge) is to try to get hold of an IDH1 vaccine on a compassionate use basis.


My immediate next step is to see the MRI results next week, but I'd be very grateful for any advice on how to proceed from here. Especially, I'd like to try and resolve the following questions:

1. Would it be unwise to not do any additional TMZ cycles? Are there any obvious chemotherapy alternatives perhaps?

2. Would the immunotherapy using liquid biopsy at IOZK clinic be a good alternative for ongoing chemotherapy cycles? Would it be advisable to start this right away (i.e. without any additional TMZ cycles), or should I do some TMZ cycles first just to hedge my bets?

3. Any other recommendations in terms of maintenance strategies. E.g. what medication(s) could make a good maintenance therapy, without concurrent chemotherapy?

Questions about Celebrex and Tamoxifen

Hi,


I am new to agents of GBM. I have following questions


1) Is it necessary or beneficial to take Celebrex in the periods between chemotherapy(TMZ) cycles?


2) We also want to add Tamoxifen to my mother's cocktail list, But we are not sure if it will be effective.


3)One study article tells me that adding folic acid will be helpful to change MGMT from unmethylated status to methylated status, Is that correct?


It would be greatly appreciated if you could share your knowledge or information about these questions.

Celebrex control edema? also question about boswilla

I come from China,this blog is wonderful to help my GBM wife.I want to start with a quick question. Stephen,i saw in the library excel that celebrex can help reduce edema,but i can not find related study in the library, do you have it?

I read the study on boswilla serrata control edema pretty well, just want to confirm is that still the best way?

My niece keep fighting with GBM. Last 3er January  did new MRI and we are waiting for the result but with the hope that the MRI were well (for some anticipated information)  ..
She has GBM no methilated and we think that temodar is not so effective in this cases. She had a second operation after first operation and treated with temodar. We read about Avastin that is very hopeful.

She started taking a non toxic cocktail of drug (curcumine, fish oil, Melatonina, PSK, Spirulina, Green te, vitamin C, D3,K2, resveratrol, etc) and Savitex with THC y CBD. She doesn't have any medical supervision about the cocktail. After a radioteraphy and little period of rest, she again is taking chemotherapy (temodar) and we think to add others drug (a little bit more toxic) that we read are good to fight against tumors, but we are scared about its interactions. We think to add Chloroquine and Celebrex (celecoxib). 

Do you think it’s a good option to join THC and CBD with Chloroquine and Celebrex?. I read about the moderate interactions between Chloroquine and Celebrex, but I didn’t read anything clear for me about THC/CBD and Chloroquine or Celebrex.
Please could you help us about your experiences? We would appreciate any  information. Thank you very much.

Celebrex and Anti-coagulants

On 10/17, my husband was diagnosed with a recurrence of GBM and was removed from his PD-1 trial.  We are now anxiously awaiting another resection and, in the meantime, are investigating potential off-label treatments like Celebrex. Unfortunately, Celebrex apparently increases the chance of bleeding when taken with anti-coagulants, which my husband must take.

I am writing to ask if you or your loved one are taking both Celebrex and Warfarin (or a similar anti-coagulant) and what your experience has been with them. I think this can be addressed by low dosing and careful monitoring, but I would prefer to proceed based on concrete experience than on speculation.

Checkpoint blockade and Cox 2 inhibitors

Since it is becoming increasingly evident that inflammation can be beneficial to enhanced immune response when using checkpoint inhibitors, I am wondering if it might be prudent to discontinue celebrex and possibly even EFA's (fish oil in this case), in an attempt to enhance the immune response.  Any thoughts on this?

First Steps

Hello,

My dad (age 64) was recently diagnosed with stage four GBM. We are still awaiting pathology information regarding MGMT, etc. He had a biopsy, but the tumor is inoperable at this time. He started chemo and radiation on Friday, but I want to start him on additional supplements and medications to improve his chances. I am sifting through the blog and more. 

Could you outline your suggestions for first steps please? I have already ran into opposition with his neuro-oncologist, so I am frustrated and overwhelmed and looking for where to start.

My name is Tanya, I'm from Ukraine, Kiev. My husband Vadim, 46 years old, has been operated glioblastoma and 28 March 2016. Now he  gets radiotherapy and temozolomide.
Please, help us in the development of a strategy for the further treatment of Vadym.
We have the results of histology and immunohistochemistry:
1.Histology.
this is a very cellular glial neoplasm consisting of pleomorphic, atypical gemistocytic astrocytes which diffusely infiltrate the cerebral cortex and subcortical white matter, forming secondary structures of Scherer. Mitotic figures, including atypical forms, are readily recognized, as well as prevascular lymphocytic infiltrates and foci of microvascular proliferation. In addition, there are large areas of confluent necrosis with early organisation and some trombotic blood vessels.
2.Immunohistochemistry:
ki-67 ( sp6)- positive tumor expression to 15 %
Bcl-2a Ab-1 (Clone 100/D5)- tumor expression weakly positive,
p53 ( clone Sp5) positive expression to 5 %/
And another part of brain (left temple)/
ki-67- positive expression to 5 %
CD 45/T200/LCA Ab-2 ( clone PD7/26/16 +2B11)- positive expression in abscess,
GFAP glial fibrillary Acidic Protein Ab-6(GFAP) positive expression.

Conclusion :
Pleomorfhic glioblastoma ( ki-67- to 15 %) with a plot of abscecc.
Our protocol:
radiation and TMZ 180 mg 1Ñ… per day.
1. Metformin 2,5 mg per day (1+0,5+1)
2.Chlorochine 250 mg Ñ… 1 per day,

Wife's Cocktail

Hello, everyone.
My 36 YO wife was diagnosed with a GBM on 11/12/2015, just four days after giving birth to our first child, debulked of 60-80% on 12/15/2015 and had the Gold Standard from 1/5/2016-1/25/2016. We have been on a ketogenic diet since 1/8/2016. She is MGMT methylated

This is what my wife was/is taking so I wanted to know everyone's thoughts if they had any:

During Radiation/Chemo
  • Zofran - 4mg/3X/day
  • Dexa - 4mg/2X/day
  • Temodar - 110mg/1X/day
  • Valcyte - 450mg/4X/daily
  • Keppra - 1000mg/2X day
  • Celebrex - 200mg/1X/day
  • Depakote - 250mg/3X/day
  • Nexium - 20mg/2X/day
  • Cannibis - 10-50mg/day as needed
  • Mushroom extract (Matake) - 600mg/day
  • Fish Oil - 1100 mg of EPA plus 120 mg of DHA (2X/day) (2 capsules)
  • Super Bio Curcumin/Turmeric - 400mg/2X/day
  • Multivitamin


After 1/25/2016 to today
  • Dexa - 4mg/morning - 2mg/night

Questions
She is getting pill fatigue so I'm wondering what I can do to reduce/add pills for the most impact:
  1. Should we increase the antioxidants (ECGC, Pomegranate, VitaminD, and Broccoli Sprout Extract)?
  2. We have DCA - is there a risk if we do that in addition to the ketogenic diet?
  3. We anticipate having access to chloroquine in 20 days, is it too late to add that?
  4. As a followup to the last question, the reason why we didn't add the chloroquine is that it's contra-indicated for Celebrex, which we felt was more important.
  5. What is considered a "low-dose" of Celebrex?
  6. Any other suggestions?

Please advise

Hello all
I was wondering if you could help me clear up something
My dad will be finishing his radio+chemo tomorrow and he is set to start high dose of chemo on the 3rd of March 5/23.
Now I am working towards a cocktail but I am unsure if I only give him the cocktail a few days before his chemo and a few days after or do I give him it continually ?
Your advise would be much appreciated

Hi all I understand the importance of cox-2 inhibitors for antiangiogenesis but I am having a tuff time trying to buy it here in the uk. Oncologist are in willing to prescribe anything I suggest and I do not want to just buy it from any website.
Has anyone had any success with getting hold of Celebrex?

Tamoxifen + TMZ + Avastin

i am taking a metronomic dose of TMZ (80 mg per day) and get Avastin every two weeks.
I have recently added Tamoxifen (currently 20 mg bid, will be increasing to 30 bid tomorrow)
Is it ok to take those three together?
