Vadym and Tania. Please, help start correctly!

Thank you, Stephen, for having added me-)
My name is Tanya, I'm from Ukraine, Kiev. My husband Vadim, 46 years old, has been operated glioblastoma and 28 March 2016. Now he  gets radiotherapy and temozolomide.
Please, help us in the development of a strategy for the further treatment of Vadym.
We have the results of histology and immunohistochemistry:
1.Histology.
this is a very cellular glial neoplasm consisting of pleomorphic, atypical gemistocytic astrocytes which diffusely infiltrate the cerebral cortex and subcortical white matter, forming secondary structures of Scherer. Mitotic figures, including atypical forms, are readily recognized, as well as prevascular lymphocytic infiltrates and foci of microvascular proliferation. In addition, there are large areas of confluent necrosis with early organisation and some trombotic blood vessels.
2.Immunohistochemistry:
ki-67 ( sp6)- positive tumor expression to 15 %
Bcl-2a Ab-1 (Clone 100/D5)- tumor expression weakly positive,
p53 ( clone Sp5) positive expression to 5 %/
And another part of brain (left temple)/
ki-67- positive expression to 5 %
CD 45/T200/LCA Ab-2 ( clone PD7/26/16 +2B11)- positive expression in abscess,
GFAP glial fibrillary Acidic Protein Ab-6(GFAP) positive expression.

Conclusion :
Pleomorfhic glioblastoma ( ki-67- to 15 %) with a plot of abscecc.
Our protocol:
radiation and TMZ 180 mg 1х per day.
1. Metformin 2,5 mg per day (1+0,5+1)
2.Chlorochine 250 mg х 1 per day,
3. Levetiracetam 1g х 2 per day ( he has no seasures),
4. Dexametaxone 2 mg ( we are going to replace it for boswellia)

My most important question is, what can i add to protocol during radiotherapy?
Are the supplements, my husband takes, permissible with radiotherapy?

  1. Other things you could consider during radiation are Depakote (another anti-seizure med), Celebrex, minocycline and a carbohydrate-restricted diet. Much of the evidence (preclinical and clinical) is summarized here:

    Some kind of COX-2 inhibitor (like Celebrex) is a good idea, as tumors produce lots of immunosuppressive, pro-invasive prostaglandin E2 (PGE2) in response to radiation treatment, and Celebrex (or other COX-2 inhibitors) could prevent that to some extent.

    Iron is generally contraindicated during cancer. Although 25mg is not a lot, I would be cautious with using it. Unless of course there is a medical reason to be taking iron, but even then I would proceed carefully.

    1. There is in vivo evidence that iron supplementation could improve the efficacy of radiation, but caution is still warranted as this hasn't been tried in humans. You are correct that iron is generally contraindicated for cancer patients.

  4. Hi, all, Stephen!
    May i ask about doses of Minocycline? In the trial https://clinicaltrials.gov/ct2/show/NCT02272270 max dose is up to 800 mg per day. So, how can i determine dose for my husband? Hi is now 115 kg. Following instruction is 200 mg 2 per day first days and after- 100 mg 2 per day, but duration is 10-15 days. Could you, please, help me with this question?

    1. In the CUSP9v3 protocol for recurrent glioblastoma, minocycline is started at a dose of 50 mg twice daily, increasing to a target dose of 100 mg twice daily (200 mg per day) for the duration of the therapy. This could be a good place to start.

    I totally understand what you fight with, i am from Romania and i think in same position, the diference is that my hubby had his last surgery in october 2014, and then we started radiotherapy and chimiotherapy, and cocktail after the radiation ,i would like to cooperare with you in finding medicine at low cost, if you don't mind.my email is melivoicu@gmail.com

    I use it for my daughter, it works good against viruses.
