Temozolomide (Temodal) dose

Hi, everybody!
may i ask about first cycle of temodal after radiation? ( firstly diagnosted).
One NO said to start on 26 day after finish radiotherapy with 480 mg per day ( 190 cm, 110 kg), another NO said to start temodal on 30-35 day after radiation with daily dose 360 mg. Can someone share your thoughts on this?

  1. Hi, my father is being treated with post radiation Temodal. The first cycle has a dosage of 150 mg/m2 and the next five cycles will be 200 mg/m2. He started 31 days after radiation. In your case the dosage should translate to 357 mg first cycle and 476 mg after that, if they're using the same dosage protocol.

  2. As Peter noted, TMZ dosage is based on body surface area.

    Plugging his height and weight into a body surface area calculator gives about 2.4 m2

    So for him, 150 mg/m2 equates to 360 mg (2.4 x 150), and 200 mg/m2 equates to 480 mg (2.4 x 200).

    I wouldn't worry about the difference between 26 versus 30-35 days, which doesn't seem significant to me.

    As far as I know the standard protocol is to use 150 mg/m2 for the first monthly TMZ cycle, and increase to 200 mg/m2 for the second and later cycles if no intolerable toxicity develops at the lower dose.

      in my country they gave from start, i just calculated with Stephen body surface calculator 160 mg TMZ, so be ferm with oncologist and told him the method of calculation , because our oncologist calculated 2 m2 for my husband , instead of 2,5 m2 body surface.

  4. Hi, Melinda!) Thank you! They know about methods of calculation, but one want to be more agressive with gbm. He propose to make full check up of Vadym functions, lab tests atc before start. And im thinking. The matter it this NO, who want to be more agressive, is open to try something else, he proposes some additional variants and he is ready to discuss. Such a doctor - a rarity. But he is ready to help in any way Vadim, even if we decide not to follow his advice on temozolomide.
