First results of cocktail trial (memantine, metformin, mefloquine, temozolomide)

I'll be working on a review of the latest brain tumor news coming from the ASCO conference currently underway.  Since this is the brain tumor cocktails blog, thought I'd post these results to start off.

Phase I factorial study of temozolomide plus memantine, mefloquine, and metformin as post-radiation adjuvant therapy for newly diagnosed glioblastoma.

The abstract gives maximum tolerated doses for doublet therapy (TMZ + one of the other drugs), triplet, and quadruplet combinations.

2 year survival rate for the entire trial population (all combintions) was 43%, similar to the 2-year survival seen in the EF-14 trial for the Optune + TMZ arm.

Since some of the combinations were probably more effective than others, it will be interesting to see the results when separated by treatment arm.

Glutamate, ROS and Endocannabinoids

At 7:40 in this video: https://www.youtube.com/watch?v=Cd5AYWX_bT4&t=1s he talks about how endocannabinoids block neurotransmitters such as glutamate. You can't see the slides which might make it more helpful but can anyone relate this is the usefulness of cannabis, especially as it relates to IDH1 mutated tumors?

I'm also trying to make sense of how ROS is thought to be related to glutamate? I thought I read that glutamate in IDH mutated tumors stops the cells from being overwhelmed by an influx of ROS and so that is why they are so glutamate hungry.

So my next thought is could a combination of cannabis and high dose vitamin c infusions, which is thought to also overwhelm the cells with ROS, be beneficial.

I have no idea if any of this makes sense. It's just overlap I'm seeing and so I thought I'd put it out there.

Memantine and Radiation

Dear all:

This is to let you know that Memantine (Nameda, an anti-Alzheimer drug) can be used to prevent radiation-induced neurocognitive decline to a certain extent. My sister's radiation oncologist recommends it, it is apparently routinely prescribed for children undergoing brain radiation.

Patients usually take it a few days before radiation, during treatment, and for six months post-treatments. My sister's side effects so far have been headache, body ache, tiredness, and nausea.

But it might be worthwhile to ask for it.

Mefloquine; Memantine; Metformin; Temozolomide

What do you think about adding the above medications to the cocktail? I was browsing trials on MDA site and ran across this one

I already take Metformin and TMZ.  Have a chance to order Mefloquine and Memantine and wondering if I should. Is Mefloquine similar to Chloroquine?
