We are not 100 % sure but I am thinking about unifontis. We need peptide vaccines because we don't have fresh tumor tissue and unifontis has peptide vaccines. Iozk dosn't have peptide vaccines and Dr. Nesselhut has only peptides for some hla types and not for all. Hallwang clinic also has peptide vaccines for all hla types..
IOZK stands for Immunological Oncology Centre, Koln (Germany). This and Nesselhut's clinic, as well as Unifontis and Hallwang, these are all cancer immunotherapy clinics in Germany.
Hi all, I'm also looking at immunotherapy and spoke to Stefaan Van Gool at IOZK as i should (hopefully) have enough fresh tissue for lysate vaccine (+pdl1 antibody if necessary). He said they the treatment plan consists of 2 vaccines and any relapse after it is treated as a ´new case´. I´m new to all this so would appreciate if anyone can shed some light on IOZK´s approach, perhaps compared to other clinics you may know about. Cheers! Mark Kinghorn
If you have a fresh tumor then maybe Iozk is a good choice but they are expensive 35-40 tys euro for 2 vaccines that includes virus and hyperthermia. If you not go for hyperthermia that saves you 2000. For people without tumor tissue I'm not sure if they are too good because they don't have peptides. The expensive vaccines they have are probably unprimed in that case. Hallwang clinic has peptide vaccines and add 2 pd and pd1 checkpoint inhibitor. If you have a fresh tumor tissue then once you ship it to storemytumor.com you can divide it into parts and do chemosensitivity testing on some of the tumor and make vaccine from the other part of it. The chemosensitivity test shouldn't be very expensive.Immune-therapy.net, unifontis, hallwang and iozk they all offer fresh tumor vaccine, hallwang does not have the newcastle virus the other 3 clinics do. Prices are : hallwang 60-70 tys euro for a few months of therapy . They give 5 vaccines in a week to save on pd1 and pd inhibitors. They administer those inhibitors for about a week I think. I'm not sure. First 5 days is 35 tys euro. Few months later they give more shots. They don't use tetanus ,no imiquimod only the expensive inhibitors .Price above includes those inhibitors. Immune-therapy as the only clinic of the 4 can do vaccine from paraffin block tissue but I was told that the such tissue is of low quality and it would not make a good vaccine. I don't know if this is truth. Immune-therapy.net offer peptide vaccines only for people with 1 hla type. Unifontis and hallwang have peptides for all HLA types. HLA test in germany is 150euro. In other countries it might be as much as 11 times more expensive.
In Hallwang clinic price of shots depends on the peptides choosen with EGFR3 being the most expensive 14 tys euro. Hallwang gives about 50-100 peptides for the prices mentioned above. Survivin is 4000. At Unifontis they told me that they give as many peptides as I like but they did not state a number. For my brothers tumor which is not EGFR 3 mutated I would certainly would like him to get the survivin peptide since it showed very good results. Stephen probably has it somewhere but I will add the link bellow. https://www.roswellpark.org/media/news/investigational-brain-cancer-vaccine-produces-strong-immunological-response
I did not ask how much is a fresh tumor vaccine in unifontis. Peptides are 15 tys euro for 6 shots when matched only to hla type. 18 tys euro when they match it to both hla and paraffin block molecular testing. Some other costs add upt to 30 tys euro for the 6 shots above spaced 3 weeks apart. Immune-therapy.net price about 4500 per a vaccine .
Unifontis states that only the first shot has to be give in Germany and the remaining shots can be given at patients house if the country laws allows transport of the vaccine. The first trip is for the HLA test. I was told that for this test all HLA loci ABC....should be tested and it should be a high resolution test. The test cost 150 euro in Germany and 11 times more expensive in my country.
So Unifontis offers personalized peptide vaccines which is a distinct type from dendritic cell vaccines pulsed with peptides. So in this case I am wondering if our efficacy boosting tricks like tetanus would be relevant with those. Peptide vaccines offered by Hallwang are also peptide vaccines which are not dendric cell vaccines pulsed with peptides.
Thanks Anna that's a lot of info there. Sadly i'm not versed enough to know what some of the terms mean or imply but its a great start so thank you. What are peoples thoughts on doing DC + newcastle virus vaccine during first round of TMZ (after radiochemo). My oncologist is very much against it however dr Van Gool at IOZK is in favour. Interested to hear your thoughts. Mark
Anna thanks from me also. As far as I see it you have nothing to lose with immunotherapy except money - it is pretty expensive. Supposedly it works best with minimal tumor residue. I was also in contact with Mr. Van Gool and he said: "it has been shown that immunotherapy targets e.g. TRP2 taking out TRP2 expressing tumor cells (TRP2 is a resistance mechanism against temozolomide)."
She is against doing anything during standard treatment lead by her. We will try for it anyway though, I'm just concerned that they only give 2 vaccines. Are you going with Dr Van Gool?
Bellow is a study comparing efficacy of PPV versus Dendric peptide Pulse vaccine. I am wondering why they say that they have simialr efficacy if the OS is so different. It seems to me that the Dendric Peptide Pulsed vaccine is better. I am wondering how it depends on the number of the peptides given.
"Many clinical trials of dendritic cell (DC)-based vaccinationsusing autologous DC and tumor-associated antigen peptideshave been carried out to assess the ability of these vaccines toinduce clinical responses in cancer patients.(51–54)Rahmaet al.(54)carried out a comparative study of DC-based vaccineversus non-DC-based authentic peptide vaccine. Twenty-oneadvanced ovarian cancer patients were divided two groups:arm A received a p53 CTL-epitope peptide with Montanidewith IL2; arm B received the same peptide-pulsed DCs withIL2. The median progression-free survival and overall survivalwere 4.2 (arm A) i 8.7 (arm B) months and 40.8 (arm A) ver-sus 29.6 (arm B) months, respectively. This study suggests thatthe simple peptide vaccination and labor-consuming DC-basedvaccination therapy are similarly effective."
Good find Anna, I'll make a point to read this one.
ReplyDeleteGood to know Anna!, where are you planning to go? IOZK or Dr Nesselhut Clinic? We are in the same page. Thanks,

I'll try to comment on this tomorrow. Time for me to shut the computer off right now.
Can somebody help me to contact Stefaan Van Gool?
ReplyDeleteHave you tried emailing IOZK?
